ABSTRACT

As fístulas oronosais adquiridas são defeitos que permitem a comunicação anormal entre as cavidades oral e nasal. Inúmeras causas podem ser atribuídas à etiologia de tal defeito. O presente trabalho visa relatar o caso de um cão com fístula oronasal adquirida que foi submetido à reconstrução cirúrgica do palato com o uso de malha de polipropileno e prótese acrílica. Após estabilização do quadro clínico e remoção manual das larvas, o paciente foi encaminhado para o desbridamento cirúrgico das lesões orais e correção do defeito no palato. A palatoplastia foi realizada com o uso da malha e resina de polimetilmetacrilato autopolimerizável. A malha foi suturada sobre o defeito palatino com pontos simples separados e fio náilon 3-0 nas extremidades da ferida. Em seguida, a resina foi preparada e, ao atingir consistência adequada, foi aplicada e modelada sobre o a malha de polipropileno com o auxílio de um descolador de Molt, sendo também suturada nas bordas da fístula com pontos simples separados e fio náilon 3-0. No mesmo tempo cirúrgico foi inserida uma sonda esofágica para alimentação enteral. Devido complicações cirúrgicas no pós operatório, o paciente foi submetido a mais duas intervenções cirúrgicas para desfecho satisfatório do tratamento. Conclui-se que associação entre a tela de polipropileno e a prótese acrílica foi uma alternativa exequível para a palatoplastia em cão.


Acquired oronosal fistulae are abnormal communication between the oral and nasal cavities. A oronasal fistula can be the result of different causes, like oral myiasis. The objetctive of these paper is to report the case of a dog with acquired oronasal fistula that underwent surgical reconstruction of the palate using polypropylene mesh and acrylic prosthesis. After stabilization of the patient ́s clinical condition and mecanic removal of the larvae, the dog was referred for surgical debridement of the oral lesions and correction of the defect in the palate. Palatoplasty was performed using a mesh and polymethylmethacrylate resin. The mesh was sutured over the palatal defect with separate simple suture and 3-0 naylon thread at the ends of the wound. Then, the resin was prepared and, upon reaching adequate consistency, it was applied and shaped over the polypropylene mesh with Molt peeler. It was also sutured on the edges of the fistula with separate simple suture and 3-0 nylon suture. At the same surgical time, an esophageal tube was inserted for enteral feeding. Due to complications, the dog underwent two more surgical interventions for a satisfactory treatment outcome. It is concluded that the association between the polypropylene mesh and the acrylic prosthesis was a practical alternative for palatoplasty in dogs after oral myiasis.

ABSTRACT

Abstract Vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (FGF-2) have the ability to increase vascular proliferation and permeability. The aim of this study was to quantify the release of two diffusible angiogenic growth factors (VEGF and FGF-2) after rapid maxillary expansion (RME). Thirty animals were randomly assigned to two groups. Control group (5 rats - intact suture) and Experimental groups (25 rats with RME) which were evaluated in different periods of treatment. Five animals were euthanized in different periods of healing at 0, 1, 2, 3, 5 and 7 days after RME. RT-PCR was used to evaluate the gene expression of angiogenic growth factors released on different periods of study. Data were submitted to statistical analysis using ANOVA followed by Tukey test and significance was assumed at a=0.05. RT-PCR showed that mRNAs of VEGF and FGF-2 were expressed in intact palatal suture tissue. mRNAs of VEGF and FGF-2 was upregulated in early periods (24 h) after RME (p<0.001 and p<0.01, respectively). The molecular levels of VEGF never returned to its original baseline values, and FGF-2 expression decreased up to day 5 (p<0.001) and suddenly increased at day 7, returning to its original level. RME increased VEGF secretion, but decreased FGF-2 secretion when compared to intact tissue. The results showed that these angiogenic growth factors are released and regulated in the palatal suture tissue after RME and could make an important contribution to the knowledge of overall reparative response of the suture tissue during the bone remodeling process.


Resumo Fator de crescimento endothelial (VEGF) e fator de crescimento de fibroblasto (FGF-2) tem a capacidade de aumentar a proliferação e permeabilidade vascular. O objetivo deste estudo foi quantificar a liberação dos dois fatores de crescimento (VEGF e FGF-2) após expansão rápida da maxilla (ERM). Trinta animais foram divididos aleatoriamente em dois grupos. Grupo Controle (5 ratos - sutura intacta) e grupos Experimentais (25 ratos submetidos a ERM) que foram avaliados em períodos diferentes de tratamento. Cinco animais foram eutanaziados em diferentes períodos de avaliação aos 0, 2, 3, 5 e 7 dias após ERM. RT-PCR foi usado para avaliar a expressão gênica dos fatores de crescimento liberados nos diferentes períodos de estudo. Os dados foram submetidos à análise estatística usando ANOVA seguido do pós-teste de Tukey com nível de significância de a=0.05. RT-PCR mostrou que os RNAm de VEGF e FGF-2 estavam expressos na sutura palatina mediana intacta. Os RNAm de VEGF e FGF-2 foram estimulados nos períodos iniciais (24h) após ERM (p<0.001 e p<0.01, respectivamente). Os nívies moleculares de VEGF nunca retornaram aos valores originais, e a expressão de FGF-2 reduziu até o dia 5 (p<0.001) e de repente aumentou até o dia 7, retornando aos níveis originais. ERM aumentou a secreção de VEGF, mas diminuiu a secreção de FGF-2 quando comparado ao tecido intacto. Os resultados mostraram que estes fatores de crescimento são liberados e regulados na sutura palatina mediana após ERM e podem ser de importante contribuição para o entendimento da resposta reparadora geral do tecido da sutura durante o processo de remodelação óssea.

ABSTRACT Objective: Evaluate the height, thickness and cortical density of the palatal bone of adults with different vertical facial patterns using Cone-Beam Computed Tomography (CBCT). Methods: This study analyzed 75 CBCTs of patients between 18 and 35 years old (45 men and 30 women). The CBCTs were classified into three groups based on their facial pattern: normodivergent, hypodivergent and hyperdivergent as determined from lateral cephalograms synthesized from the CBCTs. The height, cortical thickness and cortical density of the palatal bone were measured at 4, 8, 12, 16 and 20mm posterior to the incisive foramen, and at 3, 6 and 9mm lateral to the midpalatal suture. ANOVA with Tukey post-hoc tests were used for analysis of the data, at significance level of p< 0.05. Results: The hypodivergent pattern had a significant difference and the greatest height and cortical thickness of the palatal bone, followed by the hyperdivergent and the normodivergent patterns. No significant differences were found in minimum and maximum values of cortical density. Conclusion: The palatal bone is a favorable anatomical area to install different orthodontic temporary anchorage devices (TADs), where individuals with the hypodivergent vertical facial pattern have a higher height and cortical thickness of the palatal bone, followed by the hyperdivergent pattern and finally the normodivergent pattern. No significant differences in the cortical density of the palatal bone in the three facial patterns were found.


RESUMO Objetivo: Avaliar a altura, a espessura e a densidade cortical do osso palatino em adultos com diferentes padrões faciais verticais, utilizando a tomografia computadorizada de feixe cônico (TCFC). Métodos: O presente estudo analisou 75 TCFCs de pacientes com idades entre 18 e 35 anos (45 homens e 30 mulheres). As TCFCs foram classificadas em três grupos, de acordo com seus padrões faciais: normodivergentes, hipodivergentes e hiperdivergentes, conforme determinado na radiografia cefalométrica lateral reconstruída das TCFCs. Altura, espessura e densidade cortical do osso palatino foram aferidas a 4, 8, 12, 16 e 20 mm para posterior do forame incisivo e a 3, 6 e 9 mm lateralmente à sutura transpalatina. Os testes ANOVA e post-hoc de Tukey foram utilizados para análise dos dados, com nível de significância de p< 0,05. Resultados: O padrão hipodivergente apresentou uma diferença significativa e a maior altura e espessura cortical do osso palatino, seguido pelos padrões hiperdivergente e normodivergente. Nenhuma diferença estatisticamente significativa foi encontrada nos valores mínimos e máximos da densidade cortical. Conclusão: O osso palatino é uma área anatomicamente favorável para instalar diferentes dispositivos de ancoragem temporária. Indivíduos com padrão facial vertical hipodivergente apresentam maior altura e espessura cortical do osso palatino, seguido do padrão hiperdivergente e finalmente do padrão normodivergente. Não foi encontrada qualquer diferença significativa na densidade cortical do osso palatino entre os três padrões faciais.

ABSTRACT: There has been little discussion about the quality of life of patients with maxillary defects. This article evaluates the issues related to the condition. We performed a cross-sectional study of patients with maxillary defects from referral centers in Brazil. To avoid subject burden, a questionnaire was developed, based on questions from seven instruments, which dealt with domains and conclusions that were similar to those from other studies. The predictor variable was the patients' score for each question. The outcome measure was the presence of the best-ranked items on the questionnaire as the impact factor. Six experts assessed these items and suggested which questions to include or exclude. Patients scored each item according to its occurrence and importance. Descriptive statistics and the items' rank according to the impact factor were computed to determine whether there is a comprehensive instrument available. Thirteen patients and six professionals were included in this study. The patients' age ranged from 24 to 72 years (mean (standard deviation, SD), 50.41 (14.46) years). We obtained a 60-item instrument from the selected questionnaires and subject interviews. Only 12 (37.5 %) out of the 32 best-rated items were verified by the existing instruments, two (6.25 %) were suggested by professionals and 18 (56.25 %) were conclusions from other studies. To the best of our knowledge, this is the first study to provide many features related to the quality of life in patients with maxillary defects.


RESUMEN: Existe escasa discusión en la literatura sobre la calidad de vida de los pacientes con defectos maxilares. Este artículo evalúa los problemas relacionados con esta condición. Realizamos un estudio transversal de pacientes con defectos maxilares de centros de referencia en Brasil. Se desarrolló un cuestionario basado en preguntas de siete instrumentos, que trataba sobre dominios y conclusiones similares a las de otros estudios. La variable de estimación fue la puntuación de los pacientes para cada pregunta. La medida de resultado fue la presencia de los elementos mejor clasificados en el cuestionario como factor de impacto. Seis expertos evaluaron estos ítems y sugirieron qué preguntas incluir o excluir. Los pacientes puntuaron cada ítem según su ocurrencia e importancia. Se calcularon las estadísticas descriptivas y la clasificación de los ítems según el factor de impacto, para determinar si existe un instrumento completo. Trece pacientes y seis profesionales fueron incluidos en este estudio. La edad de los pacientes osciló entre 24 y 72 años [media (desviación estándar), 50,41 (14,46) años]. Obtuvimos un instrumento de 60 ítems de los cuestionarios y entrevistas de temas seleccionados. Solo 12 (37,5 %) de los 32 ítems mejor calificados se verificaron de acuerdo a los instrumentos existentes, dos (6,25 %) fueron sugeridos por profesionales y 18 (56,25 %) fueron conclusiones de otros estudios. De acuerdo a nuestro conocimiento, este es el primer estudio que proporciona características relacionadas con la calidad de vida en pacientes con defectos maxilares.

Antecedentes: la cirugía reconstructiva del tercio medio de la cara es compleja y variada. La vecindad anatómica con la órbita, la base del cráneo y el seno maxilar favorece la extensión tumoral del paladar a dichas estructuras, desafiando al cirujano que debe realizar una resección con intención curativa. Objetivo: obtener conclusiones sobre la supervivencia y el intervalo libre de enfermedad en cánceres palatosinusales T4a/b, sucesivamente operados durante un período de 30 años. Material y métodos: la cirugía se extendió a la órbita en el 85,2%, al cráneo en el 8,3%, al cuello en el 18,7% y a la glándula parótida en el 7,3%. La reconstrucción de partes blandas se realizó con colgajos libres en el 32,5%, musculares en el 21,6%, de vecindad en el 20,2%, musculocutáneos en el 14,2% y con piel en el 11,3%. Resultados: se produjeron complicaciones locales y generales. Estas últimas llevaron a la muerte de 4/203 ‒2%‒ pacientes. La supervivencia global a 5 años fue del 62,5% y la libre de enfermedad, del 53%, El análisis multivariado para recurrencia fue significativo en los vírgenes de tratamiento previo y para supervivencia a favor de los escamosos frente a otras estirpes histológicas. Conclusiones: en presencia de oftalmoplejía o compromiso del contenido orbitario o de ambos, la exenteración tiene indicación absoluta. La supervivencia a 5 años resulta aceptable si se tiene en cuenta que fueron solo estadios avanzados de la enfermedad. Los tratamientos previos con que concurrieron algunos pacientes fueron negativos para su evolución. La cirugía primaria desempeñó un papel esencial en la supervivencia libre de enfermedad.


Background: the problems of reconstructive surgery for the midface are variable and can be very complex. The anatomical proximity of the midface to the orbit, base of the skull and maxillary sinuses is a challenge for the surgeon who must perform a curative resection. Objective: The aim of this presentation is to report the survival rate and disease-free interval in T4a and T4b neoplasms of the palate and paranasal sinuses consecutively resected over a 30-year period. Material and methods: Surgery was extended to the orbit in 85.2%, the skull in 8.3%, the neck in 18.7% and the parotid gland in 7.3%. Soft tissue reconstruction was performed using free flaps in 32.5%, muscle flaps in 21.6%, local flaps in 20.2%, musculocutaneous flaps in 14.7% and skin flaps in 11.3%. Results: Local and general complications were reported, and 4/203 patients (2%) died. At 5 years, overall survival was 62.5% and disease-free survival was 53%. Univariate analysis revealed that lack of previous treatment was significantly associated with recurrence and squamous cell carcinoma was a predictor of survival. Conclusions: The indication of exenteration is mandatory in the presence of ophthalmoplegia or involvement of the orbital content. Survival at 5 years is acceptable, considering the advanced stages of the disease. In some patients, previous treatments were associated with adverse outcome. Primary surgery plays an essential role for disease-free survival.

Abstract Introduction Since oropharyngeal surgery alone is often insufficient to treat obstructive sleep apnea (OSA), advances have been developed in hypopharyngeal surgery. Objective To assess hyoid suspension surgery as part of amultilevel OSA surgery, also including palatal surgery. Methods The study included patients with OSA symptoms with apnea hypopnea index (AHI) > 15. They were scheduled for hyoid suspension after a nasoendoscopy during Müller maneuver and drug induced sleep endoscopy (DISE). All patients had body mass index (BMI) < 35 kg/m2. Hyoidothyroidopexy combined with tonsillectomy and palatal suspension was performed in all cases. Results The mean AHI dropped significantly (p < 0.0001) from 68.4 ± 25.3 preoperatively to 25.6 ± 9.52 postoperatively. The mean lowest oxygen (O2) saturation level increased significantly from 66.8 ± 11.3 to 83.2 ± 2.86 (p < 0.0001). In addition, the snoring score significantly decreased (p < 0.0001) from a preoperative mean of 3.4 ± 0.54 to 2 ± 0.7 at 6 months postoperatively. In regard to the Epworth sleepiness scale (ESS), it showed significant improvements (p < 0.0001) as its mean diminished from 13.8 ± 5.4 preoperatively to 5.2 ± 1.6 postoperatively. Conclusion Hyoidothyroidopexy using absorbable suture seems to produce a good outcome in treating OSA. It could be effectively and safely combined with other palatal procedures in the multilevel surgery for OSA.

Abstract Free gingival grafting, the most predictable technique to increase the keratinized gingiva, leaves an open wound on the palate and the resulting discomfort during the healing phase is a significant concern. This study was intended to evaluate the effect of topical erythropoietin on healing of the donor site. Twelve patients lacking an attached gingiva at two sites in the mandible were included. In the test group, 1 mL of gel containing erythropoietin at a concentration of 4,000 IU mL-1 was applied to the donor site, whereas the control group was treated with 2 mL of the gel alone. On the second day after surgery, the same procedure was repeated. H2O2 was used to evaluate the amount of epithelialization. Clinical healing was compared using photographs and direct examination. The EPO group showed significantly better keratinization only on day 21. Comparison of clinical healing based on direct examination revealed significantly better healing in the test group on day 28. Furthermore, inflammation in the test group was lower than in the control group on the same day. Topical application of EPO improves palatal wound healing during the third and fourth weeks after free gingival graft procedures.

A utilização de biomateriais na tentativa de reparar ou substituir tecidos perdidos não é recente e evoluiu juntamente com os avanços da medicina regenerativa visando à maior qualidade de vida da população. Na Odontologia a compreensão dos aspectos biomiméticos envolvidos após implantação de biomateriais como substitutos de tecido mole é de suma importância na prática clínica, uma vez que o enxerto autógeno de gengiva, considerado padrão ouro, possui inúmeras limitações em relação à dimensão da área a ser reparada, tempo cirúrgico e dor pós-operatória na loja doadora. Esta revisão tem como objetivo reunir estudos que propõem a utilização de substitutos ao enxerto autógeno de palato, ressaltando suas aplicabilidades, contraindicações, disponibilidade no mercado, além de resultados clínicos.(AU)


The use of biomaterials to repair or replace lost tissues is not new and has evolved with advances in regenerative medicine aimed at higher quality of life of the population. In dentistry, the understanding of biomimetic aspects after implantation of biomaterials used as soft tissue replacements is significant in practice, since the autograft gum considered the gold standard has many limitations regarding the size of the area to berepaired surgical time and postoperative pain in the donor area.This review aims to bring together studies that propose substitutes to autograft palate, highlighting its applicability, contraindications, availability in the market, and clinical outcomes. ABSTRACT Rapid maxillary expansion (RME) primarily involves the mechanical opening of the midpalatal suture of the maxillary and palatine bones. The fusion of the midpalatal suture determines the failure of RME, a common event in late adolescents and young adults. Recently, the assessment of the maturation of midpalatal suture as viewed using cone beam computed tomography (CBCT) has been introduced. Five maturational stages of the midpalatal suture have been presented: Stage A = straight high-density sutural line, with no or little interdigitation; Stage B = scalloped appearance of the high-density sutural line; Stage C = two parallel, scalloped, high-density lines that lie close to each other, separated in some areas by small low-density spaces; Stage D = fusion of the palatine bone where no evidence of a suture is present; and Stage E = complete fusion that extends also anteriorly in the maxilla. At Stage C, less skeletal response would be expected than at Stages A and B, as there are many bony bridges along the suture. For patients at Stages D and E, surgically assisted RME would be necessary, as the fusion of the midpalatal suture already has occurred either partially or totally. This diagnostic method can be used to estimate the prognosis of the RME, mainly for late adolescents and young adults for whom this procedure is unpredictable clinically.


RESUMO A expansão rápida da maxila (ERM) essencialmente consiste na abertura mecânica da sutura palatina mediana tanto nos ossos maxilares quanto nos ossos palatinos. A fusão da sutura palatina mediana determina o insucesso da ERM, um evento comum na adolescência tardia e fase adulta jovem. Recentemente, propôs-se a avaliação da maturação da sutura palatina mediana em tomografias computadorizadas de feixe cônico (TCFC), sendo apresentados cinco estágios maturacionais: Estágio A = linha sutural de alta densidade retilínea, sem ou com suave interdigitação; Estágio B = linha sutural de alta densidade, com aspecto tortuoso; Estágio C = duas linhas de alta densidade, paralelas e curvilíneas, que se aproximam em algumas regiões e, em outras, são separadas por espaços de baixa densidade; Estágio D = a fusão ocorreu no osso palatino, onde não há evidência de sutura; e Estágio E = fusão completa, que se estende anteriormente na maxila. No Estágio C, esperam-se menores efeitos esqueléticos da ERM, comparado aos Estágios A e B, visto que há muitas pontes ósseas ao longo da sutura. Para pacientes nos Estágios D e E, a expansão rápida da maxila assistida cirurgicamente (ERMAC) seria necessária, já que a fusão da sutura palatina mediana ocorreu parcial ou totalmente. Esse método de diagnóstico pode ser utilizado para estimar o prognóstico da ERM, principalmente na adolescência tardia e fase adulta jovem, período em que esse procedimento ainda apresenta-se imprevisível clinicamente.

INTRODUÇÃO: A correção cirúrgica primária do palato é de fundamental importância na reabilitação do indivíduo com fissura labiopalatina e visa tanto a restauração anatômica local, com o fechamento da comunicação existente entre a cavidade nasal e oral, como a restauração funcional do anel velofaríngeo por meio do reposicionamento dos músculos palatinos. Ao longo dos anos, as técnicas de fechamento de palato foram evoluindo progressivamente, utilizando, cada vez mais, o procedimento de reposicionamento da musculatura responsável pelo fechamento do esfíncter velofaríngeo, denominado veloplastia intravelar. Tal procedimento favorece o funcionamento sinérgico da musculatura velar e faríngea, evitando, assim, os sintomas decorrentes da insuficiência velofaríngea. No entanto, apesar de todos os esforços no sentido de conseguir o funcionamento velofaríngeo adequado, intercorrências intraoperatórias e complicações pós-operatórias imediatas e/ou tardias podem contribuir para o insucesso da palatoplastia primária e, consequentemente, levar ao aparecimento de hipernasalidade. MÉTODOS: Sessenta pacientes submetidos à palatoplastia primária com veloplastia intravelar. Intercorrências intraoperatórias e complicações pós-operatórias imediatas e tardias foram investigadas. A presença e localização de fístula ou deiscência do palato foi feita por meio de avaliação clínica. Os pacientes foram submetidos, também, à gravação em áudio de amostra de fala, as quais foram analisadas por três fonoaudiólogas. As intercorrências intraoperatórias e as complicações pós-operatórias foram analisadas de forma descritiva. A associação entre as intercorrências intraoperatórias e complicações imediatas e tardias com a formação de fístulas, bem como a associação entre a ocorrência de fístulas e deiscências com a presença e ausência de hipernasalidade, foram analisadas por meio de Teste de Fisher. RESULTADOS: Verificou-se 5% de intercorrências intraoperatórias, 20% de complicações imediatas e 13,3% de complicações tardias. O índice de fístulas foi de 16,67%. A proporção de hipernasalidade foi de 18,6%. CONCLUSÃO: A palatoplastia com veloplastia intravelar utilizada no presente estudo demonstrou ser uma técnica segura, de fácil execução, eficiente para a fala e com baixos índices de complicações.


INTRODUCTION: The primary surgical correction of the palate is of fundamental importance in the rehabilitation of individuals with labiopalatine cleft and aims for local anatomical restoration and closure of the existing communication between the nasal and oral cavities, such as functional restoration of the velopharyngeal ring through repositioning of the palatine muscles. Palate closure techniques have evolved progressively over the years and increasingly involve repositioning of the muscles responsible for closing the velopharyngeal sphincter, called intravelar veloplasty. This procedure encourages the synergistic operation of the velar and pharyngeal musculature, thereby avoiding the symptoms resulting from velopharyngeal insufficiency. However, despite efforts to achieve adequate velopharyngeal function, intraoperative events and immediate postoperative and/or late complications may contribute to primary palatoplasty failure and consequently lead to hypernasality. METHODS: Sixty patients underwent primary palatoplasty with intravelar veloplasty. Intraoperative events and immediate and late postoperative complications were investigated. The presence and location of palatal fistula or dehiscence was assessed by clinical evaluation. The patients also made an audio recording of their speech that was analyzed by three speech therapists. The intraoperative events and postoperative complications were descriptively analyzed. The association between intraoperative events and immediate and late postoperative complications with the formation of fistulae as well as that between the occurrence of fistulae and dehiscences and the presence and absence of hypernasality was analyzed using Fisher's exact test. RESULTS: Overall, there was a 5% incidence of intraoperative events, 20% incidence of immediate complications, and 13.3% incidence of late complications. Fistulae and hypernasality were found in 16.67% and 18.6% of cases, respectively. CONCLUSION: Palatoplasty with intravelar veloplasty is a safe and easily implemented technique that is efficient for speech and has low complication rates.

RESUMO Objetivo Verificar os resultados de inteligibilidade da fala de pacientes submetidos à palatoplastia primária, de acordo com o julgamento perceptivo de cinco examinadores, experientes ou não, no tratamento de fissuras labiopalatinas. Métodos Foram analisados os registros de fala espontânea, armazenados em sistema audiovisual, de 78 pacientes com fissura labiopalatina previamente reparada, de ambos os gêneros, a partir de 4 anos de idade, submetidos à palatoplastia primária. As amostras de fala foram analisadas por cinco diferentes examinadores, que classificaram a inteligibilidade de fala utilizando uma escala de 3 pontos (1=boa, 2=regular e 3=ruim). A concordância interexaminadores foi verificada por meio do coeficiente de Kappa. O sucesso da reabilitação foi analisado descritivamente quanto às proporções de pacientes, de acordo com o grau de inteligibilidade pós-operatória, determinada pelos examinadores. A significância entre as diferenças das proporções obtidas para cada categoria foi verificada pelo teste Qui-quadrado (p<0,05). Resultados A concordância entre os examinadores variou de discreta a substancial. Em mais da metade das amostras os examinadores obtiveram concordância plena. Com base nos valores médios dos julgamentos dos avaliadores, uma proporção significativa dos casos apresentou boa inteligibilidade de fala. Conclusão Verificou-se adequação da inteligibilidade de fala após a palatoplastia primária, na maioria dos casos estudados, sugerindo, assim, que esses pacientes são bem compreendidos em seu meio social. A experiência do avaliador mostrou-se uma variável importante na análise.


ABSTRACT Purpose To verify the results of speech intelligibility in individuals submitted to primary palatoplasty, according to perceptual analysis by five examiners, experienced or not with the treatment of cleft lip and palate. Methods Analysis of spontaneous speech records, registered in audiovisual system, of 78 individuals with repaired cleft lip and palate, of both genders, aged more than 4 years old, submitted to primary palatoplasty. The speech samples were analyzed by five different examiners, who scored the speech intelligibility using a three-point scale (1=good, 2=regular, and 3=bad). The interexaminer agreement was assessed by the Kappa coefficient. The treatment success was analyzed descriptively regarding the proportion of patients according to the postoperative speech intelligibility, as determined by the examiners. Significance between differences was verified by the chi-square test (p<0.05). Results The interexaminer agreement ranged from fair to substantial. Full agreement between the five examiners was observed in more than half of cases. Based on the mean values assigned by the examiners, a significant proportion of cases showed good speech intelligibility after palatoplasty. Conclusion The present outcomes evidenced adequate speech intelligibility after primary palatoplasty in the majority of cases, suggesting that these individuals are well understood in their social environment. The examiner experience presented to be an important variable in the analysis.

ABSTRACT

Mesiodens are relatively common and can cause a variety of complications. The clinician should recognize signs and problems suggesting the presence of supernumerary teeth or mesiodens, particularly aberrations in the eruptive pattern, and perform the relevant investigations. On diagnosis, each case should be managed appropriately in order to minimize complications to the developing dentition. Mesiodens is most commonly occurring supernumerary tooth located in anterior maxilla usually seen between upper two central incisors placed labially or palatally and may be erupted or impacted. The present paper describes the surgical management of palatally placed impacted mesiodens in 15-year-old female.

ABSTRACT

The aim of this study was to determine pattern of palatal fractures, review management options and find out other injuries associated with it. This retrospective study was carried out in the Department of Oral and Maxillofacial Surgery, Khyber College of Dentistry, Peshawar over a period of three years. All the patients having maxillofacial injuries were evaluated for the presence of palatal fractures. Orthopantomogram [OPG] and Para Nasal sinus [PNS] views were advised to each patient. CT scan and 3D CT scan were advised in cases with pan facial trauma and comminuted fractures. All the data were entered in specially designed proforma. A diagnosis of palatal fracture was made after correlating clinical and radiographic signs. The pattern of palatal fractures was determined along with associated facial injuries. A total of 65 cases of palatal fractures were found in this study. Majority of patients [33.8%] were in the 3rd decade of life. Regarding the pattern of fracture, majority [65%] were sagittal fractures. Most of these fractures [54%] occurred with maxillary Le Fort fractures, regarding management, 9% of the cases were treated with open reduction while 81% were managed with closed reduction techniques. Palatal fracture is often overlooked and is a cause of post op malocclusion in trauma patients. Patients with palatal bone fractures were mostly males in their 3rd decade of life. Sagittal fractures were common and the Le Fort fractures were associated with it. Closed reduction was done in most of the cases of palatal bone fractures

Velopharyngeal insufficiency in patients with cleft lip and palate is challenging for plastic surgeons. One of the treatment options is palatoplasty using buccinator muscle myomucosal flaps to extend the nasal and oral mucosa. This prospective study presents the preliminary results of the use of buccinator muscle myomucosal flaps for the treatment of velopharyngeal insufficiency in patients with cleft lip and palate. Methods: This study was performed between January 2010 and July 2012 at the Craniofacial Institute of the Hospital of SOBRAPAR. All patients with cleft lip and palate who had undergone nasofibroscopy and had severe velopharyngeal insufficiency with scar tissue accumulation in the oral cavity as observed by oroscopy, previous palatoplasty with total dissection of the soft palate, or were referred by other services were submitted to a secondary or in some cases, tertiary palatoplasty using a bilateral buccinator myomucosal flap to extend the soft palate. Results: Among 20 patients undergoing treatment, 4 (20%) were excluded from the analysis, 1 went from a very large to a large circular gap (5%), 6 went from a large circular gap to no gap (5%), 2 went from a medium to a small coronal gap (10%), and 3 exhibited no postoperative improvement (15%). Conclusions: Although longer postoperative follow-up is required, the results indicate THAT buccinator muscle flaps are reliable, reproducible, and lead to good final results.


A insuficiência velofaríngea em pacientes com fissura labiopalatal é um desafio para o cirurgião plástico e pode ser resolvida, entre outras maneiras, pela palatoplastia, com a utilização de retalhos miomucosos do músculo bucinador para alongar as mucosas nasal e oral. Este estudo prospectivo teve por objetivo fazer a avaliação preliminar do uso de retalho miomucoso do músculo bucinador bilateral para o tratamento da insuficiência velofaríngea em pacientes com fissura labiopalatal. Método: Entre janeiro de 2010 e julho de 2012, todos os pacientes do Hospital SOBRAPAR - Crânio e Face com fissura labiopalatal submetidos a nasofibroscopia e que apresentavam insuficiência velofaríngea grave, grande quantidade de tecido cicatricial na oroscopia, que já haviam sido submetidos a repalatoplastias com dissecção radical da musculatura do véu e/ou encaminhados de outros serviços foram submetidos a palatoplastia secundária, por vezes terciária, com retalho miomucoso bucinador bilateral para alongamento do véu palatino. Resultados: Dos 20 pacientes submetidos à técnica proposta, 4 pacientes foram excluídos do estudo (20%), 1 paciente passou de um gap circular muito grande para grande (5%) 6 passaram de circular grande para médio (30%) 2 de circular grande para pequeno (10%), 1 de circular grande para puntiforme (5%) 1 de circular grande para ausente (5%), 2 de coronal médio para coronal pequeno 10%) e 3 mantiveram o gap circular grande no pós-operatório (15%). Conclusões: Embora o presente estudo necessite de maior tempo de seguimento pós-operatório o retalho d músculo bucinador são confiáveis e de fácil reprodução, obtendo-se bons resultados.

ABSTRACT

Simple snoring is a social problem, one that can gravely affect the patient's married life. About 40% of men and 20% of women are affected, and it often goes along with sleep-disordered breathing. Up to now various surgical techniques have been defined such as UPPP[uvulopalatopharyngo plasty], and laserassisted uvulopalatoplasty [LAUP]. Among the surgical methods, RAUP [radiofrequency assisted uvulopalatoplasty] is a minimal invasive, an easy performed, and time and cost effective one. We designed a before and after a clinical trial. The inclusion criteria were age >18 years, complaint of nocturnal snoring, have a bed partner to assess snoring, AHI<5 events per hour in the polysomnography, malampathy score [soft palate position] one or two, an elongated uvula, grade one and two of pharyngeal webbing and patient consent was needed too. A 10-score visual analog scale [VAS] of snoring severity was completed by bed partner. All of 35 included patients underwent RAUP under local anesthesia by the same expert surgeon. After 3 months, 6 months and one year, subjective snoring decreased significantly compared to the preoperative period. The decline in VAS in 6 month compared to 3 months postoperatively, was not significant [P=0.223]. When comparing 1 year and 6 months after treatment, the VAS scores were increased, but they were not significant [From 1.8 to 1.9, P=0.78]. Three months after treatment minor complications consisted of: nasal regurgitation in 2 patients [5.7%], nasal speech in 2 [5.7%] and exacerbation of snoring in 2 [5.7%] patients.There was no major complication including mucosal laceration, uvular damage and obstruction of the airway. The rate of snoring decrease did not correlate with age, sex and BMI. Based on this study and literature review, it seems RAUP is a safe surgery, which may decrease symptoms of snoring, at least, in short-term follow-up.

INTRODUÇÃO: Diversas alterações anatômicas e funcionais são observadas nos indivíduos portadores de fissuras labiopalatinas, dentre elas anormalidades estruturais da musculatura orofaríngea, que podem causar prejuízo da função velofaríngea e, consequentemente, de funções como sucção, deglutição e fala. O presente estudo teve por objetivo avaliar a presença de fraca pressão aérea intraoral na fala de indivíduos com fissura palatina submetidos a palatoplastia primária em tempo adequado e tardiamente, e comparar se o momento do procedimento cirúrgico pode acarretar maior ocorrência do distúrbio. A hipótese é de que os indivíduos submetidos a palatoplastia primária tardiamente apresentam maior ocorrência de fraca pressão aérea intraoral em comparação àqueles que realizaram a palatoplastia primária no momento adequado. MÉTODO: Participaram do estudo 37 indivíduos de ambos os sexos, com diagnóstico de fissura de palato associada ou não à fissura de lábio, divididos em dois grupos: 1) grupo precoce (GP), composto por 22 pacientes submetidos a palatoplastia primária até o 2º ano de vida; e 2) grupo tardio (GT), composto por 15 pacientes submetidos a palatoplastia primária tardiamente, após o período de aquisição dos sons da fala. Todos os participantes foram submetidos a rotina de avaliação fonoaudiológica com intervalo de, no mínimo, 3 meses de pós-operatório. Dentre os parâmetros avaliados encontra-se a análise da fraca pressão aérea intraoral, que foi a variável considerada para este estudo. RESULTADOS: A análise dos dados possibilitou observar maior frequência de fraca pressão aérea intraoral no GT (33%) em comparação ao GP (18%). Entretanto, tal diferença não foi estatisticamente significante (P = 0,44). CONCLUSÕES: A hipótese do estudo foi rejeitada. A presença de fraca pressão aérea intraoral foi observada na fala dos dois grupos estudados, não sendo a idade na ocasião da palatoplastia primária um fator determinante.


BACKGROUND: Several anatomical and functional changes are observed in individuals with cleft lip and palate, including structural abnormalities of the oropharyngeal muscles, which can cause loss of velopharyngeal function and other functions such as sucking, swallowing, and speaking. The present study aimed to evaluate the presence of weak intraoral air pressure in the speech of individuals with cleft palate who underwent primary palatoplasty at the appropriate time or at a late stage and assess whether surgical timing may lead to a higher occurrence of the disorder. We hypothesized that individuals who undergo belated primary palatoplasty show a higher occurrence of weak intraoral air pressure compared to those who undergo primary palatoplasty at the appropriate time. METHODS: This study included 37 patients of both sexes with a diagnosis of cleft palate, with or without cleft lip, who were divided into 2 groups: 1) early group (GP), consisting of 22 patients who underwent primary palatoplasty during the second year of life, and 2) the late group (GT), comprising 15 patients who underwent primary palatoplasty at a later stage, after acquiring speech sounds. All participants underwent routine clinical assessment with an interval of at least 3 months after surgery. Among the parameters evaluated, weak intraoral air pressure was an important variable considered in this study. RESULTS: Data analysis revealed a higher frequency of weak intraoral air pressure in the GT group (33%) as compared to the GP group (18%). However, this difference was not statistically significant (P = 0.44). CONCLUSIONS: The study hypothesis was rejected. The presence of weak intraoral air pressure was observed in the speech of both groups, and the study findings showed that the timing at which primary palatoplasty was performed is not an influencing factor.

Los tumores malignos en el paladar óseo son una patología poco frecuente. Se reportan del 1 al 5% de las neoplasias malignas de la cavidad oral, donde dos tercios de las lesiones son carcinomas escamocelulares. Por lo general se presentan como una masa o ulceración no dolorosa en el techo de la boca o el borde alveolar. La invasión ósea ocurre tardíamente y las lesiones se pueden tornar exofíticas cuando no se tratan. Uno de los factores predisponentes para esta patología es el tabaquismo invertido. Las metástasis clínicas se manifiestan entre un 10 a 25%. En estadios tempranos puede ser tratado de manera efectiva con cirugía o radioterapia. Malignant tumors of the palate are an infrequent pathology. It accounts about 1 to 5% of malignancies of the oral cavity, where two thirds of lesions are squamous cell carcinomas. Usually palate tumors appear with a painless mass or an ulceration on the roof of the mouth or alveolar ridge. Bone invasion occurs late. The lesions can become exophytic when left untreated. One of the predisposing factors for this pathology is the Reverse Smoking. Clinical metastases are found between 10 to 25%. In early stages, these tumors can be succesfully treated with surgery or radiation therapy. We report a patient with and ulcerated lesion in hard palate at palatine torus level with 3 months course. Background: Oronasal communication occurs after total maxillectomy for advanced sinonasal cancers. This results in feeding, breathing and cosmetic impairment. Various methods have been described to close off the palatal defect from the oral cavity to improve the function of speech and deglutition. Aims: The object of this article is to describe our experience of preservation of palatal mucoperiosteum for oronasal separation. Materials and Methods: Retrospective review of clinical and operative records of 31 total maxillectomy patients where oronasal separation was achieved by the conventional technique of applying a maxillary obturator. The postoperative complications arising from the use of maxillary obturator for oronasal communication after total maxillectomy in these 31 patients were analysed. To avoid the complications encountered in these 31 patients we preserved and used the ipsilateral palatal mucoperiosteum for oronasal separation. This new technique was applied in 12 patients. The results are presented and compared. Results : A total of 43 patients underwent total maxillectomy for advanced sinonasal tumors. In 31 patients the conventional maxillary obturator was used for oronasal separation. Among these patients, 30 had crustation of the maxillary cavity, nasal regurgitation and cheek skin retraction in 15 each, trismus in eight, infection of skin graft donor site in seven, cheek movement during respiration in five and ill-fitting prosthesis in three. In 12 patients palatal mucoperiosteum was preserved and used for oronasal separation. The complications encountered in oronasal separation by palatal prosthesis were avoided in the modified procedure. Conclusions: We found that oronasal separation by preservation of palatal mucoperiosteum following total maxillectomy allowed excellent palatal function, prompt rehabilitation and minimal complications without compromising the prognosis.

INTRODUÇÃO: As cirurgias primárias, queiloplastia e palatoplastia, interferem na morfologia e na fisiologia do complexo maxilofacial, provocando alterações em seu crescimento nos portadores de fissura transforame incisivo unilateral (FTIU). Este estudo tem por objetivo avaliar precocemente os efeitos das cirurgias sobre o crescimento maxilofacial, por meio da relação dos arcos dentários desses pacientes. MÉTODO: Foram avaliados 45 pacientes portadores de FTIU submetidos a cirurgias primárias de lábio e palato. Foi realizado estudo comparativo de modelos dos arcos dentários decíduos em que a relação maxilomandibular foi avaliada por 2 ortodontistas, que aplicaram o índice Atack. RESULTADOS: Dentre os pacientes analisados, 44,4 por cento se encontravam nos escores 1 e 2, apresentando condições mais favoráveis de crescimento maxilomandibular. O escore intermediário (escore 3) correspondeu a 40 por cento da amostra e os escores 4 e 5, a 15,6 por cento, apresentando tendência a crescimento desfavorável. A média obtida aos 4 anos de idade foi de 2,62 + 0,98. Ao relacionar os escores 1 e 2 com outros estudos, houve diferença significativa (P = 0,023) comparativamente à série de Bongaarts, que apresentou os melhores resultados. Nos resultados para os escores 3, 4 e 5, observaram-se proporções semelhantes às de três dos estudos relacionados e melhores em relação ao primeiro a utilizar o índice Atack na avaliação de suas cirurgias primárias. CONCLUSÕES: A aplicação do índice Atack possibilitou a análise dos resultados de cirurgias primárias sobre o crescimento maxilofacial e a comparação destes com os dados obtidos por centros de referência para tratamento da FTIU.


BACKGROUND: Primary surgeries such as cheiloplasty and palatoplasty interfere with the morphology and physiology of the maxillofacial complex, causing alterations in its growth and development in unilateral cleft lip and palate (UCLP) patients. The aim of the present study was to perform a preliminary analysis of the effects of surgery on maxillofacial growth through an examination of dental arch relationships. METHODS: Forty-five patients with UCLP who underwent primary surgery for the repair of cleft lip and palate were evaluated. Comparative analysis of plaster models of dental arches was performed by two orthodontists using the Atack index. RESULTS: Some patients (44.4 percent) analyzed showed scores of 1 and 2, representing the most favorable maxillofacial growth conditions. The intermediate score (score 3) was found in 40 percent of patients, while 15.6 percent showed unfavorable maxillary growth tendencies (scores 4 and 5). The mean score at 4 years of age was 2.62 + 0.98. A correlation between scores 1 and 2 in the present study with those of previous studies resulted in a significant difference (P = 0.23) in comparison to Bongaarts' series, which obtained the best results. Our results for scores 3, 4, and 5 were similar to those of 3 related studies in terms of the percentages obtained, which were better than those of the first study that used the Atack index for the evaluation of primary surgeries. CONCLUSIONS: The Atack index enabled the analysis of the effects of primary surgery on maxillofacial growth and a comparison with the results obtained by other centers for the treatment of UCLP.

ABSTRACT

A fissura labiopalatina é o defeito facial mais facilmente encontrado e implica diversos problemas estéticos e funcionais. O uso de modelos de treinamento é uma opção recomendável no treinamento de jovens cirurgiões, principalmente em cirurgias da cavidade oral, em que o acesso cirúrgico e o manejo dos instrumentos são dificultados. O objetivo deste artigo é descrever um modelo reprodutível de treinamento em fechamento de fendas palatinas, desenvolvendo segurança e habilidade técnica nos jovens cirurgiões para realização da palatoplastia. O modelo de treinamento é constituído de cinco etapas, incluindo aulas teóricas, vivência do ato operatório e confecção do modelo de treinamento. O modelo de treinamento proposto neste artigo permite que o cirurgião se familiarize com o trabalho em cavidade pequena e profunda, aprenda a utilizar lentes de aumento e/ou microscópio, e treine incisões, dissecção e sutura em profundidade, podendo ser confeccionado manualmente e com baixo custo. O simulador proposto permite treinamento realístico em palatoplastia, podendo ser usado para o ensino da técnica cirúrgica, sendo facilmente reprodutível.


Cleft lip and palate is the most common facial defect and causes several aesthetic and functional problems. The use of training models is recommended for training new surgeons, especially for surgeries in the oral cavity where surgical access and instrument handling are difficult. This paper describes a replicable training model of cleft palate closure that helps new surgeons acquire the technical skills required to carry out safe palatoplasty surgeries. The training model is composed of 5 steps, including theoretical classes, monitoring of real surgery procedures, and the preparation of the training model. The training model proposed here allows the surgeon to become familiar with surgery in the small and deep oral cavity; to learn how to use magnifying glasses and/or microscopes; and to perform incisions, dissection, and suturing at a depth. The model can be made manually and inexpensively, and is easy to replicate. The proposed simulator provides realistic training in palatoplasty and can be used to teach surgical techniques.
